You can also download the walk registration form and fill out the top portion with your name, address, and phone number, ignoring the center portion that is walk-related, and fill in your donation amount in the bottom left portion of the form. Then either provide your credit card information on the form or include a check for the amount and fax or mail the form as necessary to the fax number/address provided.
